Cellulite (also known as adiposis edematosa, dermopanniculosis deformans, status protrusus cutis, gynoid lipodystrophy, and orange peel syndrome) is the herniation of subcutaneous fat within fibrous connective tissue that manifests topographically as skin dimpling and nodularity, often on the pelvic region (specifically the buttocks), lower limbs, and abdomen.[1][2] Cellulite occurs in most postpubescent females.[3] A review gives a prevalence of 85 to 98% of women,[4] indicating that it is physiologic rather than pathologic. It can result from a complex combination of factors ranging from hormones to heredity. Cataract Cellulitis is usually the result of a bacterial infection, which has entered the body through a cut, a graze or a bite. The bacteria spreads to the deeper layers of the skin, causing worsening symptoms. In rare cases, cellulitis can also be caused by fungal infections. Cellulite is a normal occurrence, affecting almost 90 percent of women and 10 percent of men during their lifetime. On the surface, cellulite appears as lumpy or dimpled skin. However, the process of developing cellulite is more than skin deep. It is in this layer that cellulite originates. I read this article with interest. I have just been dealing with a relevant case indicating difficulty in differential diagnosis. This was a 52 year old woman who had been in hospital for over a week under the care of general physicians with a diagnosis of a possible infected knee joint and prepatellar bursa and cellulitis of the lower leg. There was no fever at any time. The CRP was 145 mg/L and the WBC was 13.0 with a neutrophilia. Urate was normal at 0.09 mmol/L. Blood cultures were negative. Fluid was not withdrawn from the knee or the bursa. She had been seen by orthopaedic surgeons but not by a rheumatologist. She was treated with intravenous clindamycin with no significant effect, and after her discharge, still on oral antibiotics, I was called in to see her. The history was of a painful knee developing over a day to extreme pain with a lot of swelling, with no injury or obvious cause, followed within a day by painful swelling of the whole of the lower leg with skin erythema. On examination the knee and prepatellar bursa were swollen and painful, there was a tender popliteal cyst, and the lower leg was very swollen and erythematous. Voluma Patients with a history of cellulitis, particularly of the lower limbs, have an estimated recurrence rate of 8–20%.12 Patients with recurrent cellulitis should be carefully evaluated for any predisposing factors such as lower limb oedema, lymphoedema, dermatitis, tinea pedis, and measures taken to address them. Patients with three to four episodes of cellulitis per year despite addressing predisposing factors could be considered for prophylactic antimicrobial therapy so long as those factors persist.12 A randomised controlled trial of phenoxymethylpenicillin prophylaxis in patients with a history of recurrent cellulitis showed a reduced rate of recurrence in the treatment group (hazard ratio [HR] 0.55, 95% confidence interval [CI] 0.35–0.86, p=0.001). The number needed to treat (NNT) was five (95% CI 4–9).27 Eat, Drink, Play It's the MRSA Quiz! For the carriers among us, you'd be surprised that the infectious superbug is lurking on this body part!
